Personal Care Plan - Member Targeted Mail Campaign

As a West Virginia Family Health provider, you may have some members who will receive a personalized mail campaign called the Personal Care Plan. The Personal Care Plan campaign is designed to target members who have open care gaps for the following Quality Improvement measures:

  • Well-Child Visits in the Third, Fourth, Fifth and Sixth Years of Life (W34)
  • Adolescent Well-Care Visits (AWC)
  • Immunizations for Adolescents (IMA)
  • Annual Monitoring for Patients on Persistent Medications (MPM)
    • Annual Monitoring for angiotensin converting enzyme (ACE) inhibitors or angiotensin receptor blockers (ARB)
    • Annual monitoring for digoxin
    • Annual monitoring for diuretics

The Personal Care Plan encourages members to promote a healthy lifestyle and West Virginia Family Health can help coordinate an office visit with their Primary Care Doctor. The personalized mailer also creates an awareness surrounding the Member Rewards Program (Gift Cards), which are based on provider claim submissions.
